Medical Weight Management Visit Coverage Information
Will my visits at Iowa Weight Loss be covered by my insurance?
- It is your responsibility as a patient to know what your insurance covers and what exclusions you may have under your policy.
- It is also your responsibility to provide us with a copy your current insurance card so we may file a claim on your behalf.
What are some examples of questions that should I be asking my insurance to see if my visits may be covered?
- Does your insurance cover weight loss, weight management, or obesity?
- Are there specific CPT codes or diagnosis codes that are not covered under my plan?
Will the charge be the same for every visit with my provider?
- The charges selected by your provider may differ from visit to visit depending on the amount of time spent and the problems that are addressed.
Will my diagnosis be the same at every visit with my provider?
- The diagnosis code used could be different every time. Although sometimes you may feel like you are being seen for the same issue your provider may be addressing different problems during each encounter. The diagnosis code submitted to your insurance company will be taken from your providers documentation for each separate encounter. We have certified coders that adhere to national coding guidelines based off of provider documentation. The diagnosis will not be altered or changed just to satisfy insurance reimbursement as this would be considered fraudulent.
Will I be financially responsible for any non-covered or excluded services that are provided to me?
- Yes, by agreeing to receive services you are financially responsible for all unpaid or non-covered services and account balances your insurance deems as your responsibility.
What are my options prior to having services if my insurance does not cover the services I want to receive?
- We do have a self-pay option that we can offer you.
